cb42541e51 Memory: Remove the need for per-thread tear down.
Make key objects using the memory pool own their own pool and delete it,
such that there is not generic per-thread pool to manage.

4ceaab166c Memory: Move to a normal model of ownership of memory pools, for new/delete.
Addresses step 4 of #976, overlaps #916.

For each pool, now, it is newed, remembered, and freed by the same entity,
rather than having a mix (thread finalize freeing current pool) that could
lead to double freeing of the same pool. It is quite rational and simple now.

This will enable reinstalling process and thread tear down.

- extra blank lines
- tabs to spaces
- replace #include comment about no location

18b637f9dc Fix warnings/errors for strict aliasing & function prototypes
This fixes various issues related to gcc's strict-aliasing warning
by using unions. It also handles various cases hit with
gcc's missing-declarations warning.

876a0e392e Add an osinclude.h below each OS directory, redirects by platform macro
* Linux folder has been renamed to Unix, to match defines and so that it
  compiles on OS X.
* This removes the need for a per-platform include search path for the
  right OS folder

51cdd90fa8 Fix the few non-portable uses of "char" (where a -1 might be relevant): All uses of char are now either "int", "unsigned char" or char arrays for storing strings. Also, went to consistent "char* foo" coding convention. (There were only a few ambiguous uses.)

c36e1d8e51 Interface and naming improvements:
- the new C++ style interface now stands on its own, with the addition of glslang::InitializeProcess() and glslang::FinalizeProcess()
 - more "global" pool names from a decade ago are fixed to be thread names
 - StandAlone.cpp fully uses one of the old-style interface or new C++ style interface

e50441ad94 Make some portability improvements identified by Christophe: A few size_t, a couple "../Include", and a whole bunch of parenthesizing "(A && B) || (C && D)", because some compilers don't believe humans know && is higher precedence than ||.

7213324259 Reframe the preprocessor as a C++ class, with instances, removing all C code, removing all global variables. Upgrade bison version to pass a parse context on through to the preprocessor. All the basic things to make something thread safe.

bd0747d6f0 Built-in symbol tables now lazily evaluated, and driven by per version, per profile input. Got all ES 100 and ES 300 built-in symbols correct.
This includes
 - doing prescan of shader to know version/profile before parsing it
 - putting precision qualifiers on built-in ES symbols
 - getting most built-in state correct for core/compatibility/missing profile
 - adding gl_VertexID and gl_InstanceID, among other ES 300 built-in symbols
 - adding the ES 300 gl_Max/Min constants
 - accepting shaders that contain nothing but whitespace without generating an error

f0fdc53e2a Implement non-square matrices, and make a few type improvements. Cleaned up a few old issues. Added two tests.
Details
 - added all the new non-square types
 - separated concepts of matrix size and vector size
 - removed VS 6.0 comments/workarounds
 - removed obsolete concept of matrix fields
